Advanced Health Directive
Everyone should have an advanced health directive in place regardless of if you are unwell or not. This document gives you the opportunity to state your wishes should you not be able to communicate such as resuscitation, ventilation or peg feeding. It also takes the burden from your family to make decisions.

Pain and Symptom Management
Symptom management can be varied depending on the severity and treatment options. All pain management treatment that requires medication will be conducted by doctors from Jupiter Home Care.

Emotional and Spiritual Support
Living with life limiting illness can be a roller coaster. You will have good days and bad. For most people we know they don't like letting on to family they are needing some support. They try to stay strong in their presence. We are able to be your sounding board. We will be the shoulder to cry on or provide you with strategies to deal with them when they arise. This support can also extend to your family members.

Care Planning & Co-Ordination
For most people and their families they haven't got experience in palliative care planning or how to implement the right care at the right time. You may not know what services are available or some simple things to help with the journey. We can develop a palliative care plan regardless of how long you are travelling. We help navigate what is important to you at all stages, including those bucket list items you want to tick off.

End-of-life Planning
There are so many stages of your palliative care journey and they can take years to get to the end of life. When you are at this stage we can make it the most comfortable and beautiful experience possible. If this part is done right not only you are looked after but your family know what to expect and what will happen.

Grief Support
Grief is a tricky thing. It jumps up and gets us at different times without warning. Grieving is a necessary part of palliative care for both yourself and your family and friends. The one thing to remember is everyone feels and expresses grief differently. There is no right or wrong way but it can cause a very mixed basket of emotions from deep sorrow to terrible anger.

Funeral planning
Having a send off the way you want it is important. Pick your songs, request everyone wears colourful attire, what ever is important to you. We do not align with any funeral homes but rather plan well ahead of time. This also takes the pressure from your loved ones at the time.

Fee $500 for standard preparation ( extra fees will be charge for liasing with external providers or organising any items)
